Originally founded in 1994 as a human services ministry on the grounds of the Sisters of St. Francis in Sylvania, Ohio, Sophia Center is a fully accredited  counseling center for families, children and adults. Prior to its formation as a separate entity in 1994, it was part of the Chiara Center, a wholistic center that focused on the mind, body and spirit approach to well-being. 

Today, under the direction of Executive Director Sister Rachel Marie Nijakowski, OSF, PhD, the Center provides quality mental health and testing services, counseling, and various therapies for underprivileged individuals, the elderly and especially women and children. As an extension of its mission to serve the marginalized and those who need its services, a satellite office opened in October 2002 in Findlay, Ohio.
